Am I Eligible For The FloridaSDC program?

 

In order to be eligible to participate in FloridaSDC, the potential participant must meet the following criteria:

 

·         Has been diagnosed with a severe and persistent mental illness

·         Be 18 years or older, and

·         Live in Clay, Duval or Nassau County and

·         Receives Supplemental Security Income (SSI), Social Security Disability Income (SSDI), disabled veteran’s income, or any other type of disability income due to psychiatric disability; or

·         Receives Social Security Income for reasons other than psychiatric disability and meets the definition of Severe and Persistent Mental Illness as described in “CFP 155-2”; or

·         Does not receive disability income due to psychiatric disability, but has an application in process or has received such income within the last 5 years and meets the definition of Severe and Persistent Mental Illness as described in “CFP 155-2”; and

·         Is not currently legally incompetent to make financial decisions, as determined by a court; and

·         Permanently resides in the department’s Circuit 4; and

·         Is not enrolled in Florida Assertive Community Treatment (FACT); and

·         Applies for Medicaid and other income support programs for which he or she may qualify; and

·         Relies on public funds to cover costs for mental health services.

 

 

Participation is voluntary and participants may discontinue program enrollment at any time.
